The instructions on the newly revised Form SS-4, Application for Employer ID Number, will indicate which IRS Campus is assigned to their specific state.
To do this, complete IRS Form SS-4 (Application for Employer Identification Number) and file it with the nearest IRS Center.
If the SS-4 is completed with the Third Party Designee and is faxed or mailed, the EIN will be sent to the Third Party Designee as stated in (3) above.
