Ex-wife obtained US Passport for minor child without my knowledge/authorization. I became aware of this about a year ago when she and my son called from the airport where they had been prevented from leaving the country. I faxed authorization. Since then I obtained the form to obtain a US Passport for my daughter and found out that both parents signatures are required to process application. How do I go about finding out whether my ex-wife forged my signature or claimed that my address was unknown to her.
